These forms are for selected Corps members only

You may have been directed to this site for a number of reasons. Completing this paperwork correctly and quickly will ensure that you will receive your subsistence allowance, and other benefits in a timely fashion. See the bottom of this page for instructions on how to submit your completed paperwork.

Confirmation of Acceptance:
All members must complete and return the Confirmation of Acceptance (COA) included in your original acceptance email. It is possible you have received an addendum to your original COA if your terms of agreement have changed significantly.

If you received a Chase E-funds Debit Card prior to January 1st, 2011 please complete the enrollment form.
If you completed your Adult Medical Health History form within the last year, you do not need to complete this form again. If you previously served over a year ago, please complete the form again.

AmeriCorps Forms
If you are interested in enrolling in an AmeriCorps Education Award, please begin the AmeriCorps Education Award Enrollment Process.

If you are NOT interested or eligible to receive an AmeriCorps Education Award there is no need to complete the paperwork. Be sure to notify corpsinterns@thesca.org of your decision or mark this on your COA.

Returning your Paperwork
Please return them to SCA 1 of 3 ways:

• Scan the forms into your system and email them to CorpsInterns@thesca.org (preferred method)

**Unless otherwise specified: if you are participating with SCA Massachusetts AmeriCorps, SCA New Hampshire AmeriCorps, SCA Hudson Valley AmeriCorps, SCA NY Adirondacks AmeriCorps, or SCA Salmon-Challis AmeriCorps, please scan and email or fax your forms to the number above and remember to bring them with you to your site.
