Before you embark on this exciting opportunity we need you to complete some very necessary enrollment paperwork. Completing this paperwork correctly and quickly will ensure that you will receive your subsistence allowance, and other benefits in a timely fashion. All forms below must be submitted to SCA prior to your start date. Failure to submit these forms to SCA in a timely manner may delay your actual position start date. Please return all paperwork to SCA as soon as possible to be certain we can support you best. See the bottom of this page for instructions on how to submit your completed paperwork.
You will receive a link from SCA Enrollment using our digital signature system. The link will contain your required paperwork. Members must fully complete and submit these documents in a timely manner. If you choose not to fill out the forms electronically, you have the option to print, complete and fax at the bottom of each form.
Returning members:
If you received a Chase E-funds Debit Card prior to January 1st, 2011 please complete the entire enrollment form.
If you completed your Adult Medical Health History form within the last year, you do not need to complete the entire form again. Please only fill out the required sections. If you previously served over one year ago, please complete the form again.
SCA requires a copy of your Driver's License for our records. If you do not have a Driver's License, we can accept a different type of photo ID such as a Non-Driver Photo Identification Card.
If you are interested in enrolling in an AmeriCorps Education Award, we also require Proof of Citizenship. AmeriCorps classifies Proof of Citizenship as a U.S. Passport, Birth Certificate, or Permanent Resident Card. We cannot accept Social Security Cards as Proof of Citizenship.

If you are interested in enrolling in an AmeriCorps Education Award, please make sure you fully complete the AmeriCorps Agreement and AmeriCorps Enrollment forms.
If you are NOT interested or eligible to receive an AmeriCorps Education Award be sure to mark this on the AmerCorps Agreement and AmeriCorps Enrollment forms. You will still be asked to sign the documents.
Submitting a criminal background check is a required part of SCA’s selection process. SCA will provide your email address to our Third Party Vendor, easybackgrounds.com, to process a criminal background check and/or a motor vehicle check as a condition of accepting you into the Corps position. Shortly after receiving your Acceptance email message from SCA, you will receive an email from Sherri Lloyd at SCA, slloyd@thesca.org asking for information to complete the background check. Please complete the online application. Look for this email in the next few days** as it may be caught in your SPAM filter. Your information must be submitted and cleared before you can begin your position. The entire process can take up to three weeks or more.
*Note: If you are selected early to a long term position, you may not receive this email for a few weeks as the background checks expire after one year from the time you complete it.
**Returning Members: If you completed your background check within the past year, you may not receive another request to complete the background check. If your background check expires in the middle of your upcoming position, you may receive an email in the future asking you to complete it. If you have questions, please don't hesitate to call us.

Travel
Refer to your COA for eligibility and travel reimbursement amounts. If eligible, you will receive a Travel Budget Form in your SCA Enrollment packet. If you are driving to the site but do not know your destination address at the time you are completing the form, please print it to submit at a later date. *If you do not electronically sign the bottom of the form, you will receive reminders for missing paperwork. Members entering the following Corps programs are not eligible for travel reimbursement: SCA NY Hudson Valley AmeriCorps, SCA NY Adirondack AmeriCorps, SCA NH Conservation Corps, SCA MA AmeriCorps.
Corps Members who are eligible for relocation travel reimbursement and choose to fly are required to book their flights using Egencia, SCA's web-based travel vendor. Please follow the links below on how to use Egencia to book travel to/from your site.
Please book air travel as early as possible. Neglecting to do so may result in extra costs to you. Corps Members who are not eligible for relocation travel reimbursement can use Egencia but are not required to.*

**Unless otherwise specified: if you are participating with SCA Massachusetts AmeriCorps, SCA New Hampshire AmeriCorps, SCA Hudson Valley AmeriCorps, SCA NY Adirondacks AmeriCorps, or SCA Salmon-Challis AmeriCorps, please submit your forms digitally and remember to bring your printed version with you to your site. If you currently have student loans in repayment status (i.e. if you have graduated and are making monthly payments), you may qualify either for forbearance for national service or for an economic hardship deferment.
Members who enroll in an AmeriCorps Education Award and have qualified student loans – a loan that is backed by the federal government – can apply for forbearance for national service. With this special type of forbearance, after you successfully complete your service AmeriCorps will pay the interest that accrued on your qualified loans during your forbearance. Forbearance is handled directly by you, AmeriCorps, and your lender. Here’s how the process will work: Once you have been begun your position and receive your welcome email confirming your official enrollment in AmeriCorps (This will happen during your 3rd week of service.), you will create an AmeriCorps account and request forbearance electronically through the My AmeriCorps system. Members who either do not enroll in an AmeriCorps Education Award or have loans that aren’t back by the federal government can contact their lender about a possible economic hardship deferment. This process would handled directly between you and your lender.
